Target Audience
This course is designed for urologists, residents and fellows-in-training who are interested in initiating or expanding the clinical application of ablative technologies into their urologic oncology practice. The primary target audience is the urologic oncologist who is interested in both didactic and hands-on skills training for prostate and laparoscopic/image-guided percutaneous kidney cancer ablation.
Course Description/Purpose/Need
The American Urological Association (AUA) has organized a faculty consisting of nationally and internationally recognized thought leaders who have developed a focused and well-defined curriculum incorporating advanced didactic and practical hands-on training on the performance of kidney and prostate cancer ablation procedures. The recent introduction of medium-term data supporting the application of ablation procedures for both prostate and kidney cancer has continued to expand the role of ablation in contemporary urologic practice. This course is designed to teach basic and advanced concepts of ablation including practical technical training. Ablation biology, patient selection criteria and hands-on training of image-guided percutaneous and laparoscopic techniques will be emphasized. The primary objective is to help the urologist successfully initiate an ablation program and incorporate these technologies into their clinical repertoire.
Lectures will include information on the clinical biology of ablation as related to urologic oncology, indications and contraindications for patient selection, and technical tips for successfully accomplishing ablation procedures. Lectures and video presentations will detail operating room set-up and equipment, laparoscopic and image-guided percutaneous access, and step-by-step surgical techniques for kidney and prostate ablation. The lectures will be enhanced by live surgical demonstrations of ablation procedures. Practical hands-on experience on CT-guided percutaneous and laparoscopic ablation skills will be gained in the laboratory setting.
Hands-on laboratory experience will focus on skills training for both laparoscopic and percutaneous kidney ablation as well as prostate ablation. This laboratory experience will consist of three (3) dedicated and disease-specific sessions. Each participant will have an opportunity to perform laparoscopic surgery in a procedural animate laboratory practicum (Session 1). The physician will learn hands-on and practical applications of laparoscopic ultrasound, renal biopsy, and ablation probe placement for both radiofrequency (RFA) and cryoablation. Session 2 will be dedicated to prostate cryoablation utilizing anatomically correct phantoms. Lastly (during Session 3), participants will work in an interventional radiology suite and review proper technique for CT guided ablative renal procedures including steps to reduce radiation exposure and avoid complications. The focus will be on optimizing patient positioning and targeting utilizing a 64-slice CT scanner and a cadaveric model.
Course Learning Objectives After completion of this course, participants should be able to:
Discuss basic and advanced concepts of tissue thermal (hot and cold) ablation
Analyze the technical steps of kidney and prostate cancer ablation procedures
Analyze the technical steps of image-guided percutaneous and laparoscopic techniques with a critical assessment of the pros/cons of each approach
Understand the proper technique and role of tumor biopsy
Recognize that thermal ablation technologies may vary in their effect depending upon the target organ
Identify ablation biology as related to urologic oncology and patient selection
