Prostatic Adenocarcinoma, Gleason’s Grade 3

- Tumor infiltrates in and among the non-neoplastic prostatic glands
- Glands have a little more variation in size and shape, but tend to be smaller than those of grade 1 and 2
- In contrast to pattern 4, the glands in Gleason’s grade 3 are discreet and well-formed. A good rule of thumb is that if you can mentally draw a circle around each gland without running into another gland, you’re dealing with a Gleason’s grade 3.
- Variations:
- cribriform carcinoma (shown on right)
smoothly-circumscribed nodules of tumor with a cribriform architecture (or large ducts that are filled and distended with tumor in a cribriform pattern) are grade 3 as well -- as long as there is no necrosis. (The presence of central necrosis in a cribriform carcinoma raises the grade to 5).
