All applicants for the Urology Care Foundation Rising Stars in Urology Research Awards Program must register online where they can:
Electronic Registration and Submission
The Urology Care Foundation requires applicants to complete the Rising Star Application Form and Application Agreement Form and print the forms for use in the application package. All materials, including the Applicant’s complete original application package, must be submitted electronically.

The deadline for application submission is Monday, June 2, 2014!
Incomplete applications will not be reviewed. All materials listed on the checklist found within the program application must be included in order to be reviewed and considered for funding.
Multiple Applications from One Institution
Multiple applications from one institution will be accepted; however the institution is limited to only one Rising Star in Urology Research Award per cycle.
Project Termination and Change of Institution
Any personnel, location, or project status changes that occur after the award announcement or during the award period must be reported to and approved by the Urology Care Foundation.
Neither Astellas nor the Urology Care Foundation discriminates as to age, race, color, gender, religion, sexual orientation, disability, citizenship or national origin in the employment and promotion of staff or in the selection of participants in the Urology Care Rising Stars in Urology Research Awards program.
Certification of Vertebrate Animals
If activities involving vertebrate animals are planned during the research, a Letter of Approval by the Institutional Animal Care and Use Committee (IACUC) must be submitted. Letters of Approval are not required at the time of application. If you receive a Urology Care Foundation award, your award is contingent upon receipt by the Urology Care Foundation of a copy of the current IACUC approval letter.
Certification of Human Subjects
If activities involving human subjects are planned during the proposed research, applicants must provide a letter of approval from their respective Institutional Review Board (IRB). Applicants are not required to submit this letter of approval with their application. If the applicant receives a Urology Care Foundation award, the applicant award is contingent upon receipt by the Urology Care Foundation of a copy of the current IRB approval letter.
