Electronic Registration and Submission
The Urology Care Foundation requires applicants to register online to:
AUA Members can begin the application process.
Not an AUA Member yet? Create an AUA ID and begin the application process. Membership is not required however all applicants must create an AUA ID to apply for Urology Care Foundation research funding opportunities.
The application package must be SUBMITTED no later than Thursday, January 15, 2015 at 5:00 pm LOCAL TIME. Applicants should allow ample time for compiling and submitting materials. Late submissions will not be considered.
Project Termination and Change of Institution
Any personnel, location, or project status changes that occur after the award announcement or during the award period must be reported to and approved by the Urology Care Foundation.
The Urology Foundation does not discriminate as to age, race, color, gender, religion, sexual orientation, disability, citizenship or national origin in the employment and promotion of staff or in the selection of participants in the Arkansas Urologic Society Summer Medical Student Fellowship Program.
Certification of Human Subjects
If activities involving human subjects are planned during the proposed research, a letter of approval from the institutional review board (IRB) must be provided. Letters of approval are not required at the time of application. If you receive a Urology Care Foundation award, your award is contingent upon receipt by the Urology Care Foundation of a copy of the current IRB approval letter.
Certification of Vertebrate Animals
If activities involving vertebrate animals are planned during the research, a letter of approval by the Institutional Animal Care and Use Committee (IACUC) must be submitted. Letters of approval are not required at the time of application. If you receive a Urology Care Foundation award, your award is contingent upon receipt by the Urology Care Foundation of a copy of the current IACUC approval letter.
