General Application Instructions
Electronic Registration and Submission
The Urology Care Foundation requires applicants to submit their materials electronically. Applicants must link to the Arkansas Urologic Society Summer Medical Student Fellowship Program to:
The application package must be SUBMITTED no later than Wednesday, January 15, 2014 at 5:00 pm LOCAL TIME. Applicants should allow ample time for compiling and submitting materials. Late submissions will not be considered.

Incomplete applications will not be reviewed. All materials listed on the checklist must be included in order to be reviewed and considered for funding.
Multiple Applications from One Institution
The Urology Care Foundation strongly prefers not to fund multiple applications from the same research or educational institution during any single award cycle. However, if circumstances arise where no other option is available, the Foundation may consider funding more than one proposal from the same institution.
Project Termination and Change of Institution
Any personnel, location, or project status changes that occur after the award announcement or during the award period must be reported to and approved by the Urology Care Foundation.
The Urology Care Foundation does not discriminate as to age, race, color, gender, religion, sexual orientation, disability, citizenship or national origin in the employment and promotion of staff or in the selection of participants in the Herbert Brendler, MD, Summer Medical Student Fellowship Program.
Certification of Human Subjects
If activities involving human subjects are planned during the proposed research, a letter of approval from the institutional review board (IRB) must be provided. Letters of approval are not required at the time of application. If you receive a Urology Care Foundation award, your award is contingent upon receipt by the Urology Care Foundation of a copy of the current IRB approval letter.
Certification of Vertebrate Animals
If activities involving vertebrate animals are planned during the research, a letter of approval by the institutional animal care and use committee (IACUC) must be submitted. Letters of approval are not required at the time of application. If you receive a Urology Care Foundation award, your award is contingent upon receipt by the Urology Care Foundation of a copy of the current IACUC approval letter.
