Data Dictionaries

Program participants must complete research using the AQUA Registry data or the AUA Annual Census data.

Note: Diseases not listed above can be considered if patients of interest and the associated procedures and outcomes are well-defined by ICD/CPT codes. Qdata modules are AQUA Registry datasets curated for research use by Verana Health, AUA data ingestion and life sciences partner.   • There are eleven years’ worth of data, 2014 to 2024, from the AUA Annual Census. Review the questions asked on the AUA Census over the years. Note: Census project proposals must be based on published Census data and not on data that is being actively collected.

Dates to Remember

December 31, 2025

  • 8 p.m. ET — CMS deadline to apply for the 2025 Extreme and Uncontrollable Circumstances (EUC) and/or Promoting Interoperability (PI) hardship exemptions.

 
January 31, 2025

  • Last day to upload the Health Insurance Claim Form (CMS 1500 Form) and sign the Data Release Consent Forms (DRCF) via the AQUA Registry dashboard.
